Job Purpose
Assign diagnostic and procedure codes to patient records after discharge, and follow up on incomplete charts that can't be finalized. Work is done under supervision and follows department guidelines.
### **Key Responsibilities:**
**Code Patient Records**
Assign appropriate codes to discharged patient records for billing, data analysis, and compliance.
Code records within 3 days of discharge, 98% of the time
Maintain 95% coding accuracy and 98% abstracting accuracy
Follow coding guidelines and priorities set by supervisor
Input data into the electronic system
**Follow Up on Incomplete Charts**
Identify and resolve missing or incomplete information in charts needed for coding.
Contact physicians for missing diagnoses/procedures
Review charts biweekly and track deficiencies
Report missing records not found within 2 days
**Other Duties**
Perform additional tasks as assigned.

Show courtesy and care to patients, coworkers, and physicians
Follow all corporate compliance and HIPAA rules
Participate in quality improvement and cost-saving efforts
Prioritize patient and employee safety
Manage time and resources efficiently

- Education:** High School Diploma or GED preferred
- Experience:** 1 year in medical records preferred
- Other:** Must follow HIPAA and safety regulations
