The Clinical Administrative Coordinator serves as a key member of the Care Management team in Colorado. Responsibilities include collecting, reviewing and entering data; research; corresponding with Medicaid and Medicare enrollees; and completing required reports. Responsible for supporting key functions related to Medicaid and Medicare program monitoring and regulatory compliance.

Primary Responsibilities
- Develop and maintain a general knowledge of CMS requirements
- Supporting Members with physical, behavioral, and/or social needs through completion of outbound calls, assessments, and care plans
- Work collaboratively to improve the Medicare enrollee’s experience
- Understand and strictly adhere to department operating procedures
- Ensure accuracy and consistency in data entry within the Essette system, as well as MPM tracking tools
- Actively participate in department meetings and workgroups
- Meet all compliance deadlines and self-manage workload using MPM tracking tools
- Interface with key internal as necessary to complete assigned tasks
- Creates the Individualized Care Plan (ICP) using the results of the Health Risk Assessment (HRA) outreaches
- Prepares final ICPs for mailing to Medicare DSNP enrollees
- Assists with outreach to Medicare DSNP enrollees to collect Health Risk Assessment (HRA) responses as needed
- Accurately documents all work for compliance and tracking
- All other duties assigned by business

Required Qualifications
- High School Diploma/GED (or higher) or 4+ years of medical call center, care coordination, case management, or community health worker experience
- 2+ years of experience within the healthcare industry
- Intermediate level of computer skills (Microsoft Word, Excel, Outlook, and Internet) with the ability to navigate a Windows environment
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
- Reside in the State of Colorado
- 2+ years of health assessment experience
- 1+ years of experience in regulatory guidance as it pertains to the Medicare DSNP Health Risk Assessment (HRA) and Individualized Care Plan (ICP) processes
- Ability to be a positive representative of the Company both internally and externally
- Ability to always work with respect and cooperation with fellow employees and clients
- Ability to meet multiple deadlines with little direction
- Ability to multi-task
- Ability to communicate effectively, both verbal and written
- Ability to understand regulatory guidance as it pertains to the Medicare DSNP Health Risk Assessment (HRA) and Individualized Care Plan (ICP) processes
- Team player demonstrating enthusiasm and commitment to company vision and mission
- Excellent organizational skills, data entry skills, and attention to details
