Summary:
Under the direction of the Ambulatory Care Team, the Community Health Associate serves as a resource to assist patients in navigating the health care system and provides the patient with community resources so that they can obtain optimal health outcomes and remain safe in the community. The Community Health Associate is an instrumental part of a multidisciplinary team, working within the community and providing outreach to patients with complex medical and social needs.

Required: Demonstrates excellent communication skill, and proficiency in basic computer skills, including accurae keyboard entry of data into relevant computer-based databases and spreadsheets. Motivates and engages both the patients and healthcare team. Ability to work with people from diverse backgrounds and experiences and be able to openly address and acknowledge observed issues and concerns. Excellent communication skills, attention to detail and the ability to be flexible in performing a variety of tasks. Travel may be required (offices, hospitals, patients residences). Minimum Experience: three years Desired: At least 2 years Experience in community-based outreach. Progressive advancement in responsibilities in a healthcare setting such as clinic, hospital, office etc.

Responsibilities
- Assists in the identification and outreach of medically complex patients.
- Collaborates with the multidsiciplinary team to implement the care plan.
- Engages patients through telephonic outreach and/or face to face visits (facility, provider office, patient residence).
- Facilitates adherence to the treatment plan through early identification and resolution of gaps in care.
- Communicates all concerns to the clinical team and manages them as necessary, proactively seeking support and guidance.
- Participates in multidisciplinary team conference(s) in order to maximize the treatment plan.
- Provides reinforcement of the treatment plan under the direction of the care manager.
- Schedules appointments and performs clerical duties as needed, including cross coverage for Care Management Associate.
- Builds relationships within the community in order to identify available resources for patients ( i.e. DME vendors, Home Health Agencies, Area Aging office etc.).
- Performs all other duties as assigned by the Manager of Ambulatory Care Management.
- Protects confidentiality of patient information, data and intellectual property and ensures compliance with national health information projection guidelines.
- Demonstrates strong interpersonal skills and interacts effectvely with practitioners, the interdisciplinary healthcare team, community agencies, and patients and families with diverse opinons, values and religious and cultural beliefs.
- Fulfills all compliance responsibilities related to the position.
- Performs other duties as assigned.
