We are seeking a Team Coordinator for our Assertive Community Treatment (ACT) Team to lead 7-10 clinicians providing intensive community-based mental health services.
- Oversee clinical assessments, treatment planning, and discharge coordination for individuals with serious mental illness and co-occurring disorders.
- Utilize evidence-based practices such as ACT, Illness Management, Motivational Interviewing, Supported Employment, and Medication Supports to tailor treatments.
- Develop and review individualized service plans, advocate for consumers' needs, and coordinate with external agencies.
- Provide supervision, education, and consultation to team members and community partners.
- Participate in on-call rotations and emergency services as required.
- Qualifications include a Master’s in Social Work, Psychology, or related field, NH licensure preferred, with at least 1 year of relevant experience.
