Essential Duties and Responsibilities:

  • Must have current CNA license for a minimum of 2 years
  • The Field Supervisor will provide non-clinical guidance and training to home care aide staff in the field and will evaluate their performance in the field
  • Reviews the Plan of Care and recommends an RN re-assessment based on the needs of client
  • Evaluates the effectiveness of home care services by obtaining feedback from the client and/or the primary caregiver
  • Submits assessments and other required documentation outlining the services rendered as indicated in a timely manner
  • Participates in case meetings and interdisciplinary team meetings as scheduled
  • Discusses any problems concerning the clients and/or the services being provided to the clients then developing appropriate and effective solutions
  • Participates in office meetings and staff development trainings
  • Assists with performance reviews of the aides and ensures competency
  • Adheres to Federal, state and accreditation requirements
  • Maintains licensures and/or relevant certifications as required


Qualifications:

  • A current CNA certificate for a minimum of 2 years
  • Must have reliable transportation
  • Must have current driver's license
  • Current vehicle insurance coverage
  • Excellent interpersonal and coaching skills
  • Ability to exercise professional discernment and maintain professional boundaries
  • Have a positive attitude
  • Self motivated and committed to excellence
  • A passion to serve and help others live their best lives possible.
