Job Description Summary

The Patient Care Navigator is responsible for working with the Director of Market Development in educating the community regarding the care and scope of services provided by HOA. The Patient Care Navigator is the champion of Hospice of Acadiana, Inc. in the community, developing relationships with area healthcare providers and the community at large to fulfill the mission and philosophy of Hospice of Acadiana, Inc.

Essential Job Functions/Responsibilities

  • Assist in various marketing and community services.
  • Assist in planning and scheduling events.
  • Develop relationships with physicians’ offices; referral sources in hospital case management offices and nursing homes.
  • Attend marketing functions; community outreach events.
  • Share innovative ideas that may improve the company.
  • Meet weekly quota of client contracts per week.
  • Strive to produce referrals from new sources.
  • Maintain a positive attitude, conveying a standard of excellence in job performance.
  • Builds and monitors community, customer, payer, and patient perceptions of Hospice of Acadiana, Inc. as a high-quality provider of services.

Qualifications

  • Education: College graduate preferred; High School diploma or equivalent required.
  • Experience: Minimum one (1) year experience in health care, home or hospice experience preferred, or demonstrated experience in working with life limiting illness and death.
  • Special Qualifications: Ability to work well with the general public. Understands/accepts hospice philosophy, principles, issues related to end-of-life care. Good oral and written communication skills. Possession of a valid driver’s license and automobile liability.
