The Crisis Recovery Center will transform crisis care by providing 24/7 walk-in behavioral health services meant to immediately identify, triage, address, stabilize and treat persons in mental health crisis and assist those who have substance abuse withdrawal needs.

POSITION PURPOSE AND OBJECTIVES

The Care Coordinator provides care coordination and advocacy across programs both within and external to the organization through collaboration with Behavioral Health, Primary Care, and Psychiatric Service providers. The Care Coordinator manages care delivery and shares information concerning client care to achieve safer and more effective care that utilizes a trauma-informed approach that recognizes and respects consumer choice, voice and culture.

Essential Job Functions

  • Assists clients with coordinating the services developed in their care plan. Works collaboratively with behavioral health, primary care, and pharmacy staff to ensure integrated care is available to clients. Provides accurate, timely, and reliable information to the interdisciplinary treatment team assigned to the client.
  • Tracks and maintains current knowledge and shares with clinical teams of assigned clients’ admissions and discharges to facilities such as jails, hospitals, and other residential or congregate settings.
  • Ensures clients receive appointments as scheduled, assists clients with completing intake processes with providers, and conducts post appointment follow up calls.
  • Identifies medications prescribed by non CCBHC providers and shares this information with the treatment teams. Facilitates medication management by coordinating the prior authorization process and by responding to Medline requests on behalf of their clients.
  • Ensures that privacy and confidentiality of the client is maintained at all times. Obtains client consent for sharing HIPPA protected information between and with all providers in the care team. Further facilitates the sharing of said information through record obtainment and transfer as required.
  • Conducts the National Outcomes Measures Survey (NOMS) survey tool on all clients in order to obtain baseline, six month progress updates. Enters information into the EMR and SPARS.
  • Participates actively in the Quality and Utilization Management functions, including aspects of care, peer review, client satisfaction, and utilization review, standards of care, record review, and outcome indicators.
  • Ensures their clients know and understand who serves on their interdisciplinary team, each member of the teams’ role, as well as their level of involvement with their care.
  • Coordinates care across DCO’s and other providers by facilitating the intake process, care follow up, and information sharing across all providers.
  • Supplements case management and other disciplines such as peer support services.
  • Develops and provides solution focused interventions
  • Completes all necessary documentation to service delivery. Maintains required productivity expectation.
  • Attends required staff meetings.
  • Reacts to change and stress productively. Accepts/Applies instructions and supervision. Gets along with others and works as a team.
  • Performs related administrative duties and special projects.
  • Maintains proper/professional boundaries with others.
  • Has / Maintains a good on-time and attendance record.
  • Performs other duties as assigned.

Requirements

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

  • In order to perform the functions of this job, the employee must possess the following: a working knowledge of Riveon Mental Health and Recovery Policy and Procedures, including sensitivity and adherence to clients’ rights, confidentiality, and health and safety issues.
  • Must exhibit sensitivity to different cultures. Must have good time management and organizational skills.

WORKING CONDITIONS

  • The work may be provided in a variety of venues and locations including in the office, in client homes, out in various community locations and businesses. Because of the variety of environments and physical conditions that will be encountered, this individual must be able to successfully interact and navigate in all types of facilities and weather conditions.
  • This includes being able to walk, climb stairs, navigate changing surface conditions including wet, slippery, and icy grounds, and function in the proximity of electrical appliances, open flames and in areas with poor air quality including potentially smoke-filled rooms.
  • This individual may periodically be required to lift and/or carry up to 10 pounds.
  • Must be able to physically interact with clients and others in emergency situations to prevent harm to clients and others.
  • The individual in this position must be able to communicate clearly both verbally and in writing.
  • Due to the extensive computer work this individual must be able to sit for extended periods of time and operate required equipment.

Requirements/Qualifications

  • Bachelor's Degree in Psychology, Social Work, Sociology or related field and prior experience is preferred. An Associate's Degree in Human Services, Psychology, Social Work or related field and at least 3 years of full-time employment experience working directly with individuals with severe and persistent mental illness will be considered in lieu of the Bachelor's Degree.
  • Experience in the field of mental health is preferred.
  • Basic computer skills required, including familiarity with Windows and Microsoft Office.
  • The necessity for travel in this job requires that the employee possess a valid Ohio driver's license, a clean driving record, have proof of adequate personal automobile insurance, and use of a personal vehicle.
  • Must have strong communication and organizational skills.
  • Experience with EHR (Electronic Health Records) is preferred.
  • Bilingual (English/Spanish speaking) a plus.

Pay Range: $37,500.00 - $49,978.77 annual salary

Placement within the pay scale is commensurate with experience and qualifications
