Responsibilities
- Conduct comprehensive assessments of patients’ health status and needs.
- Develop and implement individualized care plans in collaboration with healthcare providers.
- Monitor patients’ progress and adjust care plans as necessary.
- Provide education and support to patients and their families regarding treatment options and health management.
- Document all patient interactions and updates in the electronic health record system.
- Participate in interdisciplinary team meetings to discuss patient cases and improve care strategies.
- Advocate for patients’ needs within the healthcare system.
- Ensure compliance with all regulatory requirements and best practices in patient care.
- Current Registered Nurse (RN) license in the state of practice required.
- Must have 2 years Acute Care Case Management/Discharge Planning.
- Strong clinical knowledge in areas such as behavioral health, emergency medicine, primary care, hospital medicine, medical-surgical nursing, and emergency room protocols.
- Proficient in taking vital signs and performing basic nursing assessments.
- Excellent communication skills with the ability to work effectively within a team environment.
- Strong organizational skills with attention to detail in documentation and reporting.
- Ability to manage multiple priorities while maintaining a high standard of patient care.
