Job Overview

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that individuals receive the necessary medical services while promoting optimal health outcomes. You will work collaboratively with a multidisciplinary team to assess, plan, implement, and evaluate patient care plans tailored to each individual’s needs. This position requires a strong background in various medical settings including behavioral health, emergency medicine, and primary care.

Responsibilities

  • Conduct comprehensive assessments of patients’ health status and needs.
  • Develop and implement individualized care plans in collaboration with healthcare providers.
  • Monitor patients’ progress and adjust care plans as necessary.
  • Provide education and support to patients and their families regarding treatment options and health management.
  • Document all patient interactions and updates in the electronic health record system.
  • Participate in interdisciplinary team meetings to discuss patient cases and improve care strategies.
  • Advocate for patients’ needs within the healthcare system.
  • Ensure compliance with all regulatory requirements and best practices in patient care.

Requirements

  • Current Registered Nurse (RN) license in the state of practice required.
  • Must have 2 years Acute Care Case Management/Discharge Planning.
  • Strong clinical knowledge in areas such as behavioral health, emergency medicine, primary care, hospital medicine, medical-surgical nursing, and emergency room protocols.
  • Proficient in taking vital signs and performing basic nursing assessments.
  • Excellent communication skills with the ability to work effectively within a team environment.
  • Strong organizational skills with attention to detail in documentation and reporting.
  • Ability to manage multiple priorities while maintaining a high standard of patient care.
