The Clinical Transformation Lead (Market Clinical Liaison) is a clinical leader who collaborates with the P3 market team to implement the strategic direction and priorities for the California market with affiliate providers. The primary objective will be to enhance the quality, safety, effectiveness, and efficiency of healthcare delivery in our market. This role will provide leadership in clinical initiatives aimed at improving patient services, quality and safety outcomes, provider alignment, engagement and satisfaction, and implementing evidence-based programs, practices, and activities that deliver exceptional quality, service, and financial results.

Essential Functions

  • In partnership with the Regional VPMA, assists the organization in advancing its mission to transform the delivery of primary care by partnering with payers and providers to drive improvements in the quality (HEDIS/STAR), clinical documentation, and cost of care for every P3 California patient.
  • Developing and managing disease management and other clinical programs to improve patient quality of life and manage healthcare costs.
  • Act as the public facing and trusted representative of P3 to the medical community.
  • Fulfill duties and responsibilities of the VPMA in his/her absence.
  • Attend payer partner joint operating committee meetings as needed.


Knowledge, Skills, And Abilities

  • Demonstrated ability to resolve complex problems effectively.
  • Excellent judgment and decision-making skills.
  • Proficiency in negotiating and handling conflict resolutions.
  • Skilled in training and educating staff at all levels, including executives and providers, and demonstrating effective collaboration skills.
  • Knowledgeable of contemporary practice issues within medicine and experienced in outcomes measurement of clinical quality and utilization.
  • Familiarity with all aspects of medical group operations.
  • Strong leadership qualities, effective communication skills, and a commitment to continuous improvement in healthcare delivery. The ability to adapt to a dynamic and rapidly evolving healthcare environment is essential.


Experience

  • A minimum of five years of experience working in primary care.
  • A minimum of two years practicing in a managed care or population health environment.
  • At least three years of experience in medical leadership.


Education

  • LPN or RN degree from an accredited school required.
  • Advanced training in healthcare administration is preferred (e.g., MHA, MBA, MMM, MPH).


Licenses

  • Must be licensed in California or obtain a license prior to start date


Travel Requirements

This position is based at our P3 Health Partner California office in Stockton, California and will travel throughout the California market to work with P3 staff and provider networks. Actual number of travel days will be based on business need as determined by the regional VPMA.

Salary range: $95K-115K base salary, robust benefits package, generous PTO
