Job Description
Play a vital role in supporting individuals as they navigate complex healthcare and social service systems. This position focuses on reducing barriers to care by providing personalized education, health system navigation, and coordination of community resources. Through a trauma-informed and person-centered approach, the Navigator helps individuals build health self-management skills, improve access to appropriate services, and address key social determinants of health. This role directly contributes to continuity of care, enhanced treatment adherence, and improved long-term health outcomes of patients living with SMI/SPMI.
Essential Duties And Responsibilities
Qualifications:
Experience navigating healthcare systems and community resources preferred.
Education
A minimum of a bachelor's degree in psychology, social work, or a related field is required.

Job Description
Play a vital role in supporting individuals as they navigate complex healthcare and social service systems. This position focuses on reducing barriers to care by providing personalized education, health system navigation, and coordination of community resources. Through a trauma-informed and person-centered approach, the Navigator helps individuals build health self-management skills, improve access to appropriate services, and address key social determinants of health. This role directly contributes to continuity of care, enhanced treatment adherence, and improved long-term health outcomes of patients living with SMI/SPMI.
Essential Duties And Responsibilities
- Identify individual needs and barriers to accessing medical care and medical treatment adherence (e.g., transportation, housing, financial, etc.)
- Develop and implement individualized care plans in collaboration with interdisciplinary care teams to meet patient’s health and wellness goals.
- Provide education to patients on chronic disease management, medication adherence, smoking cessation, and other healthy lifestyle choices using trauma-informed strategies.
- Support patients in managing their health within their home environments through Functional Support Services (FSS), including skill-building and daily living assistance.
- Deliver education to healthcare and mental health providers on trauma-informed practices and reducing stigma around mental illness in medical settings.
- Establish and maintain relationships with partner organizations to coordinate care and streamline referrals to specialty and community-based services.
- Monitor progress toward individual health goals, reassess service needs, and adjust care plans as necessary.
- Document all services in a timely and accurate manner in accordance with agency policy, Medicaid billing guidelines, and HIPAA requirements.
Qualifications:
- Ability to work collaboratively within a multidisciplinary team.
- Familiarity with trauma-informed care principles and practices.
- Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy.
Experience navigating healthcare systems and community resources preferred.
Education
A minimum of a bachelor's degree in psychology, social work, or a related field is required.
