Overview
- Join as a Contract Registered Nurse in an acute hospital setting.
- Provide care coordination and case management to optimize patient outcomes.

- Perform utilization reviews ensuring compliance with payer policies.
- Educate patients and families about care plans and discharge instructions.
- Ensure adherence to CMS standards and hospital policies.
- Participate in team meetings and performance improvement initiatives.
- Support patient advocacy and smooth transitions across care settings.
- Coordinate patient care plans ensuring effective transitions across settings.
- Collaborate with healthcare professionals to optimize patient care outcomes.
- Conduct utilization reviews to align with managed care requirements.
- Educate patients and families about care plans and available resources.
- Apply clinical guidelines for care progression and patient advocacy.
- Ensure compliance with CMS standards and regulatory requirements.
- Participate in interdisciplinary meetings and improvement initiatives.
- Provide critical thinking and problem-solving in a fast-paced environment.
- Current Registered Nurse (RN) license in applicable state(s).
- Associate of Applied Science (AAS) in Nursing required.
- Minimum 2 years of experience in acute hospital care coordination.
- BLS certification required for hospital settings.
- Preferred certifications: CCM, ACM-RN, or UM Certification.
- Strong understanding of managed care and payer environments.
- Excellent communication, critical thinking, and problem-solving skills.
- Ability to thrive in a fast-paced, self-directed environment.
