- The Nurse Practitioner conducts comprehensive risk assessments in patients' homes or via telehealth, within their scope of practice.
- Responsibilities include taking patient histories, examining, diagnosing, documenting visits, and coordinating referrals.
- Maintains patient confidentiality, observes OSHA standards, and participates in professional development.
- Qualifications include an active medical license, CPR certification, and board eligibility or certification in family medicine, with strong EHR skills.
- Preferred experience in geriatrics and teamwork skills are advantageous.
- Role involves adaptability to various environments, physical mobility, and the ability to travel up to 150 miles daily.
