Licensed Practical Nurse (LPN)

Job Summary:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at [Facility Name], a long-term care nursing facility. The LPN will provide high-quality nursing care to residents in accordance with the care plan, facility policies, and state/federal regulations.

Key Responsibilities:

• Provide direct patient care under the supervision of an RN or physician
• Monitor residents' health and report changes to the charge nurse or physician
• Administer prescribed medications and treatments accurately and on time
• Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding
• Document nursing care and resident status accurately in medical records
• Participate in the development and revision of care plans
• Collaborate with interdisciplinary team members to ensure optimal care outcomes
• Respond promptly to residents’ needs and emergencies
• Educate residents and families on healthcare needs and wellness
• Maintain a safe, clean, and positive environment for residents
  
  
  

Qualifications:

• Current and valid LPN license in [State]
• CPR/BLS certification (or willingness to obtain upon hire)
• Previous experience in a nursing home or long-term care setting preferred
• Strong clinical and communication skills
• Ability to work collaboratively and compassionately with elderly residents
