Volunteers of America Delaware Valley is seeking a reliable and compassionate Care Manager to join our Navigator Division at the Lakeland facility. This role is essential to supporting clients as they stabilize, heal, and reintegrate into the community.

Volunteers of America Delaware Valley is seeking a reliable and compassionate Care Manager to join our Navigator Division at the Lakeland facility. This role is essential to supporting clients as they stabilize, heal, and reintegrate into the community.
You’ll be part of a dedicated treatment team responsible for daily operations, client monitoring, and service coordination in a residential setting. Your attention to detail, communication skills, and care-first mindset will help ensure a safe and supportive environment.
What You’ll Do
- Conduct hourly facility rounds and monitor client census and curfews
- Manage client intake and orientation, including emergency and fire safety protocols
- Oversee meal distribution, medication monitoring, and medical appointment tracking
- Perform regular room and facility inspections, fire drills, and contraband searches
- Maintain logs, document incidents, and ensure accurate recordkeeping
- Escort or transport clients when needed
- Support compliance with cleaning protocols and operational safety procedures
- Collaborate with team members in meetings and training sessions
- Interact respectfully with clients, staff, and visitors; manage visitor log and phone system
- Uphold facility policies and support clients in fulfilling daily expectations
- High school diploma or equivalent
- At least 2 years of experience in a relevant field
- Direct experience working with individuals experiencing homelessness, substance use, or mental health challenges
- Strong written and verbal communication skills
- Valid driver’s license and ability to meet agency driver standards
- Bilingual fluency is a plus
