Join the team at the Mary J. Blige Center for Women, where you will make a meaningful impact as a Care Manager under the supervision of the Center’s Director. In this role, you will support various programs, engage with referred individuals, and provide holistic, person-centered support to empower clients and promote positive change. Sensitivity to each individual’s unique circumstances, cultural background, mental health, and substance use challenges is central to this role. The salary range is $50,000-$52,000.

Responsibilities
As a valued member of our team, your responsibilities will include, but are not limited to:
- Enrolling individuals and completing the intake and documentation.
- Developing individualized, person-centered action plans to address clients’ specific needs.
- Connecting individuals to essential community resources, and conducting follows-ups as needed.
- Meeting regularly with participants to monitor/document progress and assist in addressing barriers and goal progression.
- Building and maintaining relationships with community organizations to facilitate resources access.
- Other duties as assigned.
We are seeking candidates who possess the following qualifications:
- Bachelors degree in Social Work or a related field
- Familiarity with community resources and case management practices for high-risk populations
- Bi-lingual Spanish proficiency preferred
- Strong interpersonal, engagement, and communication skills
