Case Manager 034 - Edison NJ

Salary: $45,000 per year

Key Responsibilities

• Provide case management services to individuals and families residing in the Ozanam Family Shelter and Scattered Site PSH.
• The job includes screening, assessment, service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers, life skills teaching, transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies.
• Utilization of HMIS case management software.
  
  

Requirements:

• Bachelor’s Degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred.
• At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients required.
• This job requires on-call coverage and evening hours.
• Experience with conflict resolution and mediation.
• Bilingual a plus.
• Ability to work independently.
• Must have valid driver’s license with acceptable driving history.
