Job Summary

The HEZ Outreach and Engagement Specialist will work to provide outreach opportunities for patients assigned to the Health Equity Zone panel and community residents who lack access to basic needs and information of services. Build and operate social media outlets to reach a wide range of community members in the Cranston and Warwick communities. Help educate residents on HEZ and HEZ projects, work with local partner organizations to outreach residents of Cranston and Warwick.

Work Schedule Demands

  • Full-time position, 35 hours per week
  • Due to the nature of the work and level of responsibility, this individual maintains a flexible schedule, which may go beyond a regular workweek.
  • Grant-funded temporary position with possibility for extension
  • May be required to work at any CCAP location

  • Provide outreach/in-reach to residents of Cranston and Warwick. Provide educational services available in both communities.
  • Create and outreach on social media platforms. Engage with stakeholders to help boost HEZ’s social media platforms and educate community services available to them.
  • Attend meetings to share and obtain information for outreach platforms and to contribute with new ideas for HEZ’s program.
  • Provide structured resident education through letters, phone, face-to-face encounters, and internet platforms. Engage in follow-up conversations and offer assistance to individuals.
  • Distribute outreach materials to community members, partner organizations and businesses to build coverage option awareness.
  • Develop, discover, and attend community events to promote the HEZ’s programs and opportunities.
  • Collaborate with various local organizations to build awareness of the HEZ’s program and build referral linkages.
  • Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
  • Accurately provide required reporting to track goal achievement and client satisfaction.
  • Assist in the development and implementation of organizational outreach and engagement initiatives.
  • Work to effectively develop and nurture relationships with a diverse group of stakeholders
  • Work to work independently and coordinate multiple tasks
  • Work to enthusiastically communicate with potential enrollees
  • Read and interpret documents
  • Write routine reports and correspondence
  • Other duties as assigned.

Requirements

REQUIRED QUALIFICATIONS:

  • CHW certified or currently obtaining (preferred)
  • Must be able to travel with use of a personal vehicle and have proof of vehicle insurance
  • Must possess the ability to speak effectively before groups and actively engage the public in outreach situations
  • Must possess the ability to interact respectfully with diverse cultural and socio-economic populations
  • Oral and written communication skills at a level typically acquired through completion of a bachelor’s degree program in public health, health education, communications or related program, An equivalent combination of education and experience will be considered.
  • Knowledge of the health and human services infrastructure
  • Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications, and Microsoft operating system
  • Provide/maintain all required immunizations and/or vaccinations
  • Complete all required background checks

This position requires prolonged sitting, some bending, stooping, and stretching. It requires eye-hand coordination, and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment. It also requires a normal range of hearing and eyesight to record, prepare, and communicate appropriate reports. In addition, this position requires travel between sites and the ability to interact with others in both small and large group settings. At times, this position may include periods of extended physical activity, such as walking/standing at community events and/or moving and distributing educational materials. Work is partially performed in normal, pleasant office environment. A portion of work time will be spent performing in-field outreach and enrollment assistance activities. Duties will require travel throughout the health organization’s service area. Travel expenses will be reimbursed.

Salary Description

$19.00 per hour
