Arkansas Provider Coalition (APC) provides Care Coordination for Summit Community Care, helping individuals with developmental disabilities and/or behavioral health diagnoses maintain their health and independence. Working with Arkansas Medicaid, APC ensures patient-centered care through service referrals, follow-ups, and navigation. The care process includes assessment, care planning, medication management, service coordination, and monitoring adherence to ensure timely access to quality, cost-effective care.

Position Summary
The Care Coordination Supervisor is responsible for overseeing the Care Coordinators, ensuring compliance with DHS and APC/Summit Community Care policies and procedures. This supervisory role includes managing a small caseload of members with intellectual, developmental, and/or behavioral health disabilities to ensure they receive superior care and support. Key duties include addressing service challenges, managing updates to caseloads, and conducting evaluations. The role also involves auditing, emergency support, complaint resolution, and assisting in recruitment, hiring, and onboarding.
Key Responsibilities
- Foster a positive team culture by empowering and coaching Care Coordinators.
- Provide development opportunities through training and constructive feedback.
- Conduct quality one-on-ones and address barriers to service delivery.
- Manage and update caseloads, ensuring effective caseload management.
- Handle member transfer requests diligently.
- Serve as a backup for Care Coordinators during their absence.
- Review individual cases for risk mitigation and improvement strategies.
- Promote team engagement and address care coordination complaints.
- Assist in emergencies and conduct periodic audits for documentation accuracy.
- Collaborate with the QA department to uphold service quality.
- Ensure deadlines are met and track key metrics.
- Complete performance evaluations and develop improvement plans.
- Assist in recruiting and onboarding new Care Coordinators.
Qualifications
- Education: Preferred: Bachelor’s degree in social work, psychology, sociology, or a health-related field.
- Experience: Minimum of 2 years in a supervisory role and 2 years as a Care Coordinator for individuals with developmental disabilities or behavioral health diagnoses; 3-5 years is preferred in Managed Care or Arkansas PASSE roles.
- Skills: Strong leadership, team-building, problem-solving, and time management skills are required, along with expertise in communication, mentoring, and strategic planning.

Hiring Hourly Range: $52,811 - $60,985 annually. The final offer will be determined based on the candidate's level of education, relevant experience, and overall fit for the role.
