Must be able to coordinate the delivery of patient care; completes required documentation, participates in patient/family teaching and discharge planning, effectively utilizing the Nursing process.
Provides leadership by working cooperatively and effectively with physicians, ancillary staff and other administrative and support personnel in maintaining the standards of professional nursing practice in the multidisciplinary clinical setting.
Maintains quality delivery of patient care by completing patient assignments in compliance with the Nursing process and the Arizona Nurse Practice Act, The Joint Commission, State regulations and established Hospital Policies and Procedures.
Essential Functions
To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation.
Provides patient care to Wound Care Center patients and handles patient intake and discharge processes as assigned and according to the hospital and nursing standards.
Assist in vein clinic as assigned after Healogics Vein Clinic training completed
May also serve as Case Manager to a group of patients as assigned. This entails: Performing the initial nursing assessment, communicating findings with the assigned physician, assessing the wound stage and measurements, documenting care plans, evaluating the patient' status in regards to desired outcomes, and identifying patient care situations that require intervention
Participating in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of the patient care.
Performs hyperbaric oxygen therapy (HBO) related duties as assigned, upon completion of the Healogics hyperbaric training. Collaborates as needed with other healthcare providers, Wound Care Center Providers, the Program Director and the Medical Director regarding clinic and patient needs.
May function as a Documentation Assistant (scribe) in accordance with Healogics policy. Performs other duties as required.
Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.
Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.

Minimum Qualifications
- Associate's Degree in Nursing Required. Bachelor's or Master's Degree preferred.
- An Active Arizona RN License or Compact License required.
- BLS within 30 days of hire.
- 1 year of Acute Care experience preferred.
