About The Role
We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide compassionate, proactive primary care to residents in skilled nursing and long-term care facilities. Responsibilities
- Deliver on-site primary care services in skilled nursing and long-term care facilities
- Conduct physical exams and comprehensive medical histories
- Diagnose, treat, and manage both chronic and acute conditions
- Order, review, and interpret diagnostic tests and lab results
- Prescribe and adjust medications as needed
- Create and update individualized treatment and care plans
- Collaborate with nurses, facility staff, and interdisciplinary teams
- Educate patients and families on treatment plans, prevention, and wellness strategies
- Maintain a supportive and respectful environment for residents
- Active and unrestricted NP, PA, or MD license
- Georgia state license in good standing (required)
- Minimum 6 months of experience in long-term care or skilled nursing
- Excellent clinical judgment, critical thinking, and communication skills
- Genuine passion for elder care and patient-centered healthcare
