Description
Under the Cancer Screening Navigation Programming, the Health Education and Access Coordinator will help assess individual and community health education needs related to Prostate Cancer; plan, implement, and evaluate effective health education programs; coordinate health education services; serve as a resource person in health education. The goal of the Health Education and Access Coordinator is to provide people who are at high risk of Prostate Cancer with tools that allow them to prevent developing life-threatening health issues.

Under the Cancer Screening Navigation Programming, the Health Education and Access Coordinator will help assess individual and community health education needs related to Prostate Cancer; plan, implement, and evaluate effective health education programs; coordinate health education services; serve as a resource person in health education. The goal of the Health Education and Access Coordinator is to provide people who are at high risk of Prostate Cancer with tools that allow them to prevent developing life-threatening health issues.
RESPONSIBILITIES:
- Develops and maintains relationships with community members and leaders to assist in gaining program support, referrals, and case assistance to help identify individuals at high risk of Prostate Cancer within the community.
- Performs street and community outreach for Prostate Cancer Education program promotion and case finding. Attends community related events in Bronx County.
- Conducts individualized and/or group health education around Prostate Cancer and support coordination to connect individuals to their Primary Providers for Shared Decision Making.
- Maintains chart for each encounter; clinical care supervised by collaborating clinician.
- Conducts follow-up with patients as indicated.
- Assesses individual and community needs for health education on an ongoing basis.
- Works with the Care Management Coordinator to develop/select/adapt and implement a health curriculum and promotion materials to be delivered to community members.
- Evaluates effectiveness of curriculum and revise as needed with implementation of current best practices.
- Maintains records and files.
- HS Diploma or equivalent required
- Bachelor's degree in social work, psychology, public health, health education or related field preferred
- 1-2 years of case management, health education or relevant experience required
- Basic computer and internet navigational skills required
- Computer literacy with Windows-based operating systems and MS Office applications (Word, Excel, Outlook, PowerPoint) required
- Familiarity with electronic health records (EHR) technology preferred
- Knowledge of social services, community-based organizations, and other resources in the NYC area
- Lived experience with Prostate Cancer or in the high-risk group for Prostate Cancer, preferred
- Bi-lingual Spanish preferred
