Job Summary

The Home Health Clinical Intake Specialist will facilitate and assists in all needed information to properly schedule new patient referral. The Clinical Intake Specialist optimizes patient care by processing all aspects of outpatient referral requests from outside facilities. Compiles all pertinent patient records for consultative visit including but not limited to, H&P, relevant laboratory and diagnostic testing, pathology reports, and other data in accordance with physician preferences and disease state profiles.

Qualifications

2 year / Associate Degree in Nursing (Required)

4 Year / Bachelor's Degree In Nursing (Preferred)

Work Experience

4+ years RN Experience (Required)

4+ years Home Health Intake (Preferred)

4+ years Home Health Management or Market Analytics (Preferred)

4+ years Home Health Business Development

Licenses and Certifications

Required Certifications/Licensures: Registered Nurse (RN) with current Georgia license, Basic Life Support (BLS)

Validate physician/clinic information collected by Medical Records for patients referred to Cancer Center.

Assist with gathering outside medical records and pertinent history.

Develop as well as maintain tracking metrics and quality indicators for intake process.

Must demonstrate advanced skills in extracting and analyzing patient records to ensure information at first visit is complete, current and accurate.

Establishes and maintains positive working relationships with current and potential referral sources.

Prepare patient profile/summary and document answers to significant relationship questions for Intake Nurse Navigator.

Understand key barriers, needs, concerns and what is most critical when preparing patients medical records according to diagnosis.

Self-motivated and able to work independently or in a team environment.

Attention to detail and high degree of accuracy in all work products critical for this position.

Additional Duties

Adheres to the hospital and departmental attendance and punctuality guidelines.

Performs other duties as required and completes all job functions as per departmental policies and procedures.

Maintains current Knowledge in present areas of responsibility to include any specialty certification requirements (i.e., self-education, attends ongoing educational programs).

Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.

Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills.

Wears protective clothing and equipment as appropriate.
