Overview
- Serve as a Registered Nurse in a hybrid role, combining remote and field-based responsibilities in Queens.
- Collaborate with interdisciplinary teams to deliver personalized care to high-needs populations.
- Engage patients and caregivers in proactive health management and disease education.
- Utilize evidence-based guidelines to close gaps in care and improve outcomes.
- Conduct in-home, virtual, or telehealth assessments to address patient needs effectively.
- Respond to clinical escalations promptly, providing triage and care coordination services.
- Contribute to program design and process improvement initiatives through team-based collaboration.
- Lead strategies to enhance clinical practice and deliver business value.
- Create a safe and inclusive work environment while fostering collaboration within the team.
- Conduct assessments to develop nursing care plans tailored to patients and caregivers.
- Track outcomes and reassess patient needs to ensure effective care delivery.
- Utilize care pathway templates to manage patient conditions and risk levels.
- Provide real-time responses to clinical escalations via phone, virtual visits, or home visits.
- Coordinate care, including patient navigation and interdisciplinary collaboration.
- Engage patients in managing their health, medications, and follow-up appointments.
- Follow evidence-based guidelines to close gaps in care and promote in-network services.
- Document care coordination activities in the electronic medical record system.
- Participate in team-based rounds to support program development and improvement.
- Bachelor of Science in Nursing (BSN) required; MSN or graduate-level healthcare degree preferred.
- Minimum of 1 year of clinical experience; 2+ years preferred.
- Unrestricted RN licensure in the state of hire; eNLC license preferred.
- Proficiency in Microsoft Office and electronic medical record systems.
- Basic Life Support (BLS) certification required.
- Experience in care management within health plans, hospitals, or home health settings.
- Strong organizational and analytical skills to manage patient care effectively.
- Ability to adapt priorities and collaborate with interdisciplinary teams.
- Unrestricted driver’s license required for field-based responsibilities.
