Description
The CHC Clinical Coordinator is a member of the Community Health Center (CHC) on the West Campus of ORMC. This position is responsible for the clinical management of the CHC programs and staff that implement them. This is a true hybrid position that merges clinical with administrative in a hospital outpatient setting, as well as working with our inpatient population and staff. Participation in data gathering and reporting is expected, with strong emphasis on accuracy and quality. Data may be gathered through reports and/or a manual tracking system utilizing Excel and surveys throughout our system.
CHC Clinical Coordinator is Chair of the Diabetes Advisory Committee, made up of local providers and leaders, and responsible for reporting to maintain Accreditation for programs housed within the CHC. Responsibility for the continuous development of staff and curriculum utilized for education in the department is critical, maintaining standards of care and best practices, and keeping current with changes in the healthcare industry.
The CHC Clinical Coordinator will be an expert in the field of Diabetes Education and hold a Certified Diabetes Educator certification within 2 years of hire into position. This position is the contact and resource for all things regarding diabetes management for our outpatient, inpatient, staffing and community populations. Obtaining certification as a Lifestyle Coach is required to be more effective in teaching patients and staff and will be required within 2 years of hire into position.
Patient Education and support services are interactive, collaborative, and ongoing. A process through which people with, or at risk of chronic disease gain the knowledge and skills needed to modify behavior and successfully self-manage their disease. The process incorporates the individual’s needs, goals, and life experiences, while guided by evidence-based standards. The overall objectives are to support the patient’s decision-making, self-care behaviors, and problem-solving skills using active collaboration with all involved to improve clinical outcomes and quality of life. This position requires a flexible schedule, including days, evenings, and Saturdays. Schedules may vary from week to week.
