Under limited supervision, the primary focus of the Social Services Coordinator is to support residents by working with the intradisciplinary teams to coordinate services for residents to enhance their daily living, psychosocial wellbeing and overall satisfaction. This includes services available at Valle Verde and throughout the greater Santa Barbara community. Creating needed services and programs, including resident education programs.
Full Time Benefitted, Monday to Friday
$25 to $33 per hour depending on experience and background

Essential Functions

Work Duties
- Assesses resident needs based on social services history, resident's health, psychological and social needs;
- Develops a Resident Care Plan based on the above assessment;
- Visits and interviews residents when requested by team, physicians, residents or family members to determine recreational/activity history, needs, and interests, as well as social services' needs;
- Assists families with issues or concerns relating to caring for a resident;
- Works with residents to develop measurable goals and monitor success;
- Completes documentation regarding social service needs as required by regulations;
- Coordinates, facilitates and organizes educational programs for residents regarding health, safety, and quality of life concerns;
- Advocate for Meet Ups and other supportive, life enriching programs;
- Establishes relationships with individual residents, their families and neighborhood groups;
- Serves as liaison with appropriate social and health agencies and service providers in the community;
- Meets new residents and provides teaching regarding campus services;
- Monitors residents in transition; Village stay or hospital stay to ensure needed services are in place for resident success at discharge;
- Monitors residents receiving hospice services and ensures appropriate services are in place;
- Assists with transitions to other levels of living; under the guidance of the Lodge, Grove or Village teams;
- Refers and links the residents of the community to local service providers, such as, case management, personal assistance, homemaker, transportation, counseling, visiting nurse, preventive health screening/wellness, and legal advocacy;
- Educates residents, and families on service available;
- Manages the provision of supportive services where applicable;
- Liaise with Welcoming committee to help residents build informal support networks with other residents, family and friends;
- Works with residents, families and outside agencies when need for a higher level of services has been identified;
- Report any suspected cases of abuse, neglect or exploitation of a resident to appropriate agency;
- Report problems, concerns and changes in condition to the appropriate Valle Verde department and immediate supervisor;
- May set up volunteer support programs with service organizations in the community;
Education –
- A Bachelor’s Degree or equivalent work experience in social services, psychology, or recreation therapy; or working with elders.
- 1 to 2 years of experience, preferably in a geriatric setting; or any equivalent combination of training and experience which provides the required skills, knowledge and abilities.
- RCFE Certification required, may obtain after hire.
