Description

Job Summary:

The Help Me Grow (HMG) Case Manager responds and communicates with all referrals received through email, website, or call line requests. The Case Manager will be assigned to work the call line providing support and information regarding services for young children who may have developmental, behavioral, socialization and/or learning challenges.

Essential Functions

  • Answer and respond to all calls and referrals, obtain and document information regarding reason for referral, and make recommendations for educationally and culturally appropriate support and services.
  • Obtain, compile, and maintain a directory of resources available to infants and children in Merced County.
  • Screen children for developmental delays using the Ages and Stages Questionnaire/Ages and Stages Questionnaire: Social Emotional (ASQ/ASQ:SE) as needed, translating the results and review referral options with the parent. This may be accomplished via online ASQ/SE system.
  • Serve as a link to and maintain collaborative relationships with community providers.
  • Promote, through outreach efforts, early identification and referral of children with delays. Offer the HMG Centralized Call Line as a resource to assist agencies, professionals, and parents.
  • Accurately collect, complete, enter and/or submit all necessary program documentation and data information.
  • Provide assistance to other members of the team, as needed, to ensure the smooth operation of the program/agency. This may include supporting and assisting them with their caseload of families.
  • Explain center processes, procedures and services; support families in the development and implementation of family centered care plans; and follow up with the family to ensure they have accessed the appropriate services.
  • Communicate in a courteous manner.
  • The position requires the ability to meet with children/families in various locations in the community. In order to meet this expectation staff are required to have reliable transportation for local travel.
  • Maintain, manage, and process child/family records and information in a confidential manner that meets HIPAA and other applicable confidentiality regulations.
  • Adhere to EPU standards, policies and procedures.
  • EPU facility hours are Monday – Friday; 8:00 AM – 5:00 PM; the position requires staff to be available for non-traditional work hours. Therefore, position hours may vary, which may include evening or weekends.
  • Must perform other duties as assigned by the HMG Supervisor, Director or Chief Executive Officer.

Requirements

Knowledge, Skill and Experience:

Education: Must have a Bachelor’s Degree in Child Development, Social Work or closely related field.

Skills: Must have strong communication skills both written and verbal; able to make independent decisions within guidelines, accept responsibility, dependable, reliable, time management, committed, highly motivated, possess a professional code of ethics, and professional demeanor, use good judgement, logic skills, and be self-confident.

Experience

  • 2+ years’ experience working with and/or assessing children with special needs and their families, and referring them to community service providers.
  • Must be proficient in obtaining and handling sensitive information from parents in a compassionate, confidential and professional manner.
  • Must be proficient in collaborating with staff and families in developing and implementing multidisciplinary family centered treatment plans.
  • Must be proficient in working with diverse populations.
  • Must be knowledgeable of community resources available to families with young children.
  • Must be knowledgeable of Word, Excel, Microsoft Office, Windows and Outlook.
  • Hold valid California Driver's license. Possess a good driving record with no serious violation for the past two-year period, and be able to provide proof of current automobile liability insurance.

Physical Demands

Typical Working Condition: Position works from the EPU facility and to meet with families at their place of residence or at a community based location. The position requires the ability to sit and work on a computer for extensive periods of time. Position requires fingering (fine dexterity), repetitive hand and wrist motions.

Equipment Used: Computers, printers, telephones, and fax machines. Requires operating of a vehicle for local travel.

Essential Physical Tasks: Must be able to sit for long periods of time, working with a keyboard, computer screen.
