We are seeking a Registered Dietitian (RD) to join our interdisciplinary care team. This role plays a key part in improving the nutritional health and quality of life for our residents in a long-term care setting.
Key Responsibilities:
- Conduct ongoing nutritional assessments for residents based on medical and dietary needs.
- Develop and update individualized nutrition care plans.
- Collaborate with the interdisciplinary team, including Nursing, Pharmacy, Therapy, and Food Service departments.
- Document assessments and interventions accurately in resident charts and MDS.
- Monitor residents’ weight changes, food intake, lab values, and medication interactions.
- Provide nutrition education to residents and families.
- Review and revise menus to ensure nutritional adequacy and resident satisfaction.
- Participate in care plan meetings and interdisciplinary team discussions.
- Assist in policy development, meal service evaluations, and sanitation inspections.
- Conduct staff training and in-service education related to nutrition and food service.
- Follow facility protocols related to fire safety and emergency procedures.
- Registered Dietitian (RD) credential required
- Experience in long-term care or similar healthcare setting preferred
- Strong clinical nutrition knowledge
- Proficiency in basic computer skills
- Passion for working with the geriatric population
- Annual negative TB skin test required
- Knowledge of therapeutic diets and menu planning
- Ability to interpret lab values and clinical indicators
- Familiarity with nursing home regulatory requirements
- Strong communication and documentation skills
- Experience conducting staff education and in-service trainings
