We are looking for a compassionate and skilled nursing professional to join our collaborative healthcare team. This role plays a vital part in delivering high-quality medical care and supporting the overall well-being of our patients. The ideal candidate will be responsible for coordinating medical services, administering treatments, and working closely with clinical and medical staff to ensure continuity of care.

Key Responsibilities

• Coordinate care across medical and psychiatric services
• Administer vaccines and long-acting injectable medications
• Perform routine health screenings and lab draws
• Provide patient education and basic first aid as needed
• Maintain accurate and up-to-date patient records
• Collaborate with a multidisciplinary team including physicians, nurse practitioners, physician assistants, RNs, medical assistants, care managers, and administrative staff
• Participate in staff education and training initiatives
  
  

Qualifications

• Current LPN License in the State of Arizona.
• Must be at least 21 years of age or older.
• If agency driver, possess valid driver’s license with acceptable driving record including but not limited to a serious traffic violation (ie. Reckless Driving; at fault accident; felony speeding) or greater than 2 traffic violations in prior 39 months and reliable transportation.
• CPR and First Aid certification – training will be provided if candidate does not have active certification.
• Series of Hepatitis shots or declination of same.
• Proof of good health by initial Physical examination and negative TB test.
• Occasional lifting, carrying, pushing, or pulling a maximum of 50 pounds as needed.
• Ability to make independent decisions.
• Proven ability to work within a collaborative clinical team environment.
• Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.
• Must obtain and maintain a Fingerprint clearance card and pass a background check.
  
  

Qualifications Preferred

• Bilingual English/Spanish.
• Previous Psychiatric nursing experience.
