Job Summary
The Patient Navigator plays a vital role in supporting patients through the complex landscape of health coverage and social services. This position focuses on assisting individuals and families with enrollment in the Health Insurance Marketplace, Medicaid, Medicare, and Disability programs, while also connecting them to essential community resources. The Navigator ensures that patients receive equitable access to care and services, in alignment with the mission of a Federally Qualified Health Center (FQHC).

This position focuses on assisting individuals and families with enrollment in the Health Insurance Marketplace, Medicaid, Medicare, and Disability programs, while also connecting them to essential community resources. The Navigator ensures that patients receive equitable access to care and services, in alignment with the mission of a Federally Qualified Health Center (FQHC).
Qualifications And Requirements
- Associate or bachelor’s degree in social work, public health, or related field preferred.
- Minimum 1–2 years of experience in health insurance enrollment, case management, or community outreach.
- Experience with health insurance enrollment, case management, or community outreach.
- Knowledge of Medicaid, Medicare, Marketplace plans, and Disability programs.
- Strong interpersonal and communication skills, with cultural competency and empathy.
- Bilingual (English/Spanish or other relevant language) preferred.
- Familiarity with EHR systems and basic computer proficiency.
- Insurance Navigation & Enrollment
- Assist patients with applications and renewals for Marketplace plans, Medicaid, Medicare, and Disability benefits.
- Provide education on coverage options, eligibility criteria, and deadlines.
- Maintain up-to-date knowledge of federal and state insurance programs and policies.
- Community Resource Coordination
- Identify and refer patients to local services such as housing assistance, food programs, transportation, and behavioral health support.
- Build and maintain relationships with community organizations and service providers.
- Patient Advocacy & Support
- Serve as a liaison between patients and healthcare providers to reduce barriers to care.
- Help patients understand medical bills, coverage denials, and appeals processes.
- Support underserved populations, including non-English speakers, individuals with disabilities, and those experiencing homelessness.
- Documentation & Compliance
- Accurately document patient interactions and referrals in the electronic health record (EHR).
- Ensure compliance with HIPAA, FQHC guidelines, and federal reporting requirements.
- Completes other duties as assigned.
- While performing the duties of this job, the employee is regularly required to talk and hear.
- This position is very active and requires standing, walking, bending, kneeling and stooping all day, but may also involve long periods of sitting or computer-based work.
- This employee must frequently lift and/or move items over 30 pounds.
- Must be able to lift, carry and handle equipment, supplies and other work site materials according to position requirements.
