Job Description Summary

Provide clinical and operational leadership to the MOMs (Management of Maternal Diabetes) program, a clinical program aimed at improving care for individuals with diabetes in pregnancy. Manage MOMs clinical support staff and serve as liaison to both internal and external stakeholders.

Job Description

Job Duties:

20% - Serve as clinical leader and manage all aspects of the MOMs clinical program, a multidisciplinary program that includes obstetric and endocrine care. Work closely with MDs and other clinical providers and staff to ensure success of the MOMs patients. Develop and implement strategies to improve patient outcomes. Suggest strategies to improve care coordination and improve outcomes. Provide in-person clinic support as directed both in the downtown Charleston clinics and outreach locations. Work alongside Nurse Navigator, Scheduler, Community Health Worker, and other MOMs support staff to optimize patient experience and outcomes.

20% - Collaborate with leaders from multiple agencies to develop and sustain the MOMs clinical program. This may include establishing relationships and contracts with internal and external vendors. Work with statewide agencies and vendors to promote and optimize care for the MOM program. Draft and execute contracts in support of the clinical program. Work with Division Administrator and Departmental Procurement as it relates to the procurement needs of the MOMs clinical program. Primary liaison to both internal and external agencies to achieve program goals and objectives. This may include governmental and non-governmental entities.

15% - Primary representative for communication, meetings, etc, with the sponsor (BCBS Foundation and Diabetes Free SC). Participate in bi-monthly planning meetings with sponsor, MUSC, and other SC MOMs sites. Organize and participate in meetings as need with sponsor, MUSC, and other sites/vendors. Conduct ongoing planning to estimate annual cost of sustaining all components of SC MOMs program into the future without external funding supplementation.

15% - Manage timekeeping, annual performance evaluations, and all HR related items for assigned direct reports. All management of staff must be done in compliance with divisional and departmental policies as well as state requirements.

10% - Oversee data management and submission of timely reports to funder. Manage data analyst and ensure data quality and integrity. Work alongside Division Administrator and PI to submit grant requested reports, including verbal and written, in compliance with grant reporting requirements. Manage non-payroll grant and program expenditures ensuring compliance with grant guidelines as well as MUSC and state procurement guidelines.

10% - Effectively communicate with multiple levels of leadership to include divisional leadership, clinic managers, and other stakeholders to the MOMs clinical program. Serve as primary point of contact with the IRB and management of protocols, paperwork and other regulatory related requirements to maintain MOMs clinical program compliance.

10% - Prepare and submit all PATH forms related to MOMs and tele-MOMs. Daily monitoring and actively work the clinic phone line, voicemail, rightfax, inbasket, solarity, etc. Schedule, reschedule, and cancel patients as needed. Manage referrals for MOMs and tele-MOMs. Manage MOMs patient panels including accurate scheduling, optimizing provider density, obtain patient medical records, etc.

Preferred Knowledge, Skills And Abilities

  • Knowledge of EPIC encouraged.
  • Healthcare clinical experience preferred.
