Key Responsibilities
- Conduct comprehensive assessments and evaluations of patients in their homes.
- Develop and implement personalized care plans based on patient needs.
- Manage and stabilize chronic and acute medical conditions.
- Provide high-quality medical care and support to patients and their families.
- Collaborate with other healthcare professionals to ensure optimal patient outcomes.
- Maintain accurate and timely documentation of patient interactions.
- Valid Nurse Practitioner license in New York State.
- Board certification is required.
- Must be enrolled as a Medicaid provider.
- Previous experience in home healthcare or similar settings preferred.
- Strong clinical assessment and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Rate: $100 - $150 per visit, depending on visit type
