Care Coordination & Oversight
Description
- Complete morning and noon medication passes on Mondays, Wednesdays, and Fridays.
- Conduct safety and appearance round on all Memory Care (MC) rooms: Ensure resident care is completed. Confirm room cleanliness and appropriate storage of ADL supplies. Verify resident appearance s upports dignity and cleanliness standards. Perform med cart audits to ensure medication storage compliance. Ensure discontinued medications are either returned to the pharmacy for credit or destroyed per protocol. Monitor medication rooms and carts for organization and cleanliness.
- Ensure MC residents are engaged in meaningful activities at all times.
- Monitor residents for any changes in condition or care needs and escalate to RCD/ARCD as appropriate.
- Review YARDI and QuickMAR to ensure:
- Resident photos are updated.
- Face sheets contain current insurance and demographic information.
- Check emails and voicemails daily; follow up as appropriate.
- Meet with the RCD/ARCD each morning to address priorities and any time-sensitive concerns.
- Lead daily huddles with MC staff to communicate updates, shift changes, and resident concerns.
- Review and sign off on the MC Med Tech-to-Med Tech communication logs and Change of Shift notes daily. Follow up on any outstanding items and communicate key updates during stand-up meetings.
- Manage the daily staffing schedule:
- Ensure all shifts are appropriately covered.
- Review upcoming schedules and identify gaps or variances.
- Notify RCD/ARCD of any staffing issues or training needs.
- Ensure weekend schedules (Friday–Sunday) are confirmed by Thursday.
- Create and update daily assignment forms for Assisted Living (AL) and Memory Care (MC). Ensure all assigned staff sign their assignments daily.
- Update the RCD on attendance, vacation requests, tardiness, and other staffing-related concerns.
- Participate in nursing staffing calls as assigned.
Care Coordination & Oversight
- Assist the Resident Care Director (RCD) in planning, coordinating, and overseeing resident care services.
- Monitor daily care activities to ensure residents receive appropriate and person-centered care.
- Participate in move-in assessments, ongoing evaluations, and care plan updates.
- Communicate changes in residents’ health conditions to the RCD, physicians, and families as needed.
- Provide guidance, direction, and support to Resident Care Associates and Med Techs.
- Monitor staff performance and adherence to care standards; report concerns to the RCD.
- Assist with training and onboarding of new care staff.
- Help manage staffing schedules and respond to staffing needs or changes.
- Maintain open, respectful communication with residents, families, and staff.
- Act as a liaison between residents, families, nursing staff, and other departments.
- Respond promptly to resident or family concerns and escalate appropriately
