Job Overview – Post-Acute Care Physician (Internal or Family Medicine):
Compensation: $350,000 – $400,000/year
Location: Danbury, CT
Schedule: Monday to Friday, 7:00 AM – 3:00 PM
Deliver high-quality post-acute care with our client as a Physician (Internal or Family Medicine) in Danbury, CT. Provide medical services in nursing homes, skilled nursing facilities, and other post-acute settings, managing complex cases and collaborating with interdisciplinary teams to improve patient outcomes. Responsibilities as the Post-Acute Care Physician:
- Patient Care: Deliver comprehensive medical care in nursing homes, skilled nursing facilities, and post-acute settings.
- Complex Case Management: Oversee patients with chronic illnesses, post-hospitalization needs, and serious conditions.
- Care Coordination: Work with nursing staff, therapists, and other healthcare professionals to optimize care plans.
- Rounding & Documentation: Conduct rounds, complete progress notes, and maintain compliant medical records.
- Medication Oversight: Prescribe and monitor medications for safety and effectiveness.
- Communication: Explain care plans and treatment options to patients and families with clarity and compassion.
Qualifications for the Post-Acute Care Physician:
- Licensure: Must hold an active Physician’s Medical License in the State of Connecticut, along with required DEA and CSR licenses.
- Experience: 2+ years in post-acute care, geriatrics, hospitalist medicine, or hospice care, with nursing home experience strongly preferred.
- Technical Skills: Proficiency in electronic medical record (EMR) systems and familiarity with regulatory compliance requirements in post-acute care.
- Industry Knowledge: Deep understanding of geriatric medicine, palliative care principles, and interdisciplinary care coordination.
- Skills & Attributes: Strong clinical judgment, excellent communication skills, and a patient-centered approach with a commitment to improving quality of life for medically complex patients.
