Summit Home Care is a RN-owned and operated, Joint Commission Accredited, NYS licensed, home care agency that services all 5 Boroughs in NYC and Long Island. We are seeking Registered Nurses to join our agency who have the drive and desire to help people.

RN / Registered Nurse Job Responsibilities:

  • Perform health assessments for patients in their home to assess a patient’s health care status and needs at start of care and then every 6 months and as necessary
  • Initiate the plan of care and makes necessary revisions as patient status and needs change
  • Document patient history, symptoms, assessments, tests, and care according to guidelines
  • Supervise paraprofessional staff on patient plan of care
  • Be a Care Manager for your patients
  • Educate patients and their families on proper home health care strategies and procedures
  • Reporting all issues and concerns to the Director of Patient Services
  • Complete documentation accurately and submit timely to agency
  • Provide the highest quality care in accordance with DOH regulations


RN / Registered Nurse Requirements:

  • Must possess a current, valid NYS RN license and Registration
  • Government Issued ID, Social Security Card & Work Authorization
  • 2 Reference Letters
  • Recent Physical
  • TB Screening (Quantiferon required)
  • Rubella & Rubeola titer lab work
  • Excellent communication skills
  • Strong critical thinking skills
  • Able to function independently with minimum supervision
  • Dedicated to providing the best patient care possible
  • Ability to travel to patient’s homes


Location:

Visits to be completed in 5 boroughs and Nassau
