Duties and Responsibilities:
- Inputs data elements into multiple electronic databases.
- Abstracts information in a timely manner within established time frames.
- Uses various software applications.
- Keeps abreast of regulatory/accrediting changes in a timely manner.
- Conducts searches on the internet for regulatory and accreditation information.
- Educate oneself using various methods provided such as web-based sessions, regularly scheduled abstraction clinics, and frequently asked questions forum.
- Provides education to the Medical Staff and Medical Center staff regarding analysis and interpretation of the data element requirements and the outcome of data abstracted.
- Protects the confidentiality of the medical record as mandated by law, professional standards, and the Medical Center’s policies.
- Assumes responsibility for cross-checking quality, accuracy and completeness of abstracts.
- Data abstractions shall achieve as close to 95% as possible but no lower than 85% for more than 1 quarter.
- Abstraction productivity should be as close to equivalent for the number of abstractors as possible (i.e. 3 abstractors, as close to 33% as possible).
- Notifies Cardiac Services Manager whenever time frames cannot be met.
- Performs additional quality studies and duties as directed.
- Assists with regulatory surveys or other needs as identified.
- Other miscellaneous duties and responsibilities as required for maintaining the services provided by the department.
Knowledge
- Comprehensive knowledge of medical diagnostic and procedural terminology
- Understanding of cardiac disease processes, anatomy and physiology as necessary
- Knowledge of federal and state regulations and requirements which pertain to patient care information
- Basic computer knowledge and database management
- Has a working knowledge of aspects of patient care provided in the facility
- Able to communicate effectively in English, both verbally and in writing
- Is a motivated independent individual who can organize workload so that all functions are completed appropriately and timely
- Skilled in data analysis and interpretation
- Able to interpret criteria for review and abstract data element requirements as defined, including but not limited to various cardiac accreditations.
- Ability to read handwritten and transcribed documents in the medical record, interpret information, and enter complete and accurate data into a computer system
- Ability to use designated reference materials

Education
- Graduate of Accredited School of Nursing and/or B.S.N. preferred
- Five years of recent acute care experience and knowledge of cardiac background preferred.
- Current clinical licensure in State of California, RN preferred
- Ability to adhere with AVMC Attendance and Punctuality Policy.

- Ability to adhere with AVMC Paid Time Off (PTO) Policy.
- Ability to adhere to the department dress code.

- Work is performed both in an office and nursing unit environment
- Must be able to make rounds within Medical Center and conduct patient/family interviews as well as conduct education throughout Medical Center as needed.
- Must be able to use standard office equipment, sit, and perform occasional bending and twisting.
- Ability to lift and carry 10 lbs.
- Potential exposure to biohazards-blood borne infectious waste and ability to use PPE.
