Position Summary

The IDT works as a part of the collaborative care team of healthcare professionals led by a Department of Health primary care provider to support adults with mental health and substance use disorders served in the medical home and assessed and determined to be in need of and will benefit from a low intensity level of care as determined by the LOCUS. The IDT is responsible for coordinating and leading Wraparound staffing services, utilizing natural supports, community supports, and resources from other community partners or professional agencies. The IDT Facilitator works as part of a team with a counselor, a case manager, and other primary care treatment team members, ensuring that the program staffing's follow the evidence-based practice of Wraparound. The IDT Facilitator ensures that safety and risk issues involving the client are identified and mitigated, and that each client has an individualized, strength-based treatment plan developed collaboratively during the IDT sessions. The IDT facilitator is responsible for ensuring that the level of care being provided is aligned with the LOCUS assessment. IDT staffing's are facilitated at intervals not less than every 15-days for individuals who have been assessed and determined to be in need of and will benefit from a low intensity level of care as determined by the LOCUS level 1 or 2 as part of Health Care for the Homeless (HCH), and PCHP. This position is both office and field based and reports to the Supervisor of Risk Management Staffing's. Directions for Living is an essential services provider. All Directions for Living Team Members must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.

Education/ Experience Requirement

  • Bachelor's Degree in Social Work or a related area of study from an accredited university
  • Two years of experience working with the homeless population and/or persons struggling with severe and persistent mental illness and/or substance misuse disorders
  • Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment. Preference will be given to individuals certified as a Wraparound Facilitator in high fidelity Wraparound.
