Job Summary

We are seeking a Physical Therapist or Occupational Therapist contractor to join our team at New Beginning HCSP LLC.! We are a Home Community Services (HCS) and Texas Home Living (TxHmL)provider offering care and support services to Texans with intellectual dissabilities. As a physical therapist or occupational therapist you will be working closely with clients with intellectual dissabilities. You will be responsible for the evaluation, therapy planning, direct treatment, staff/caregiver training, equipment recommendations, documentation, and participation in IDT meetings, while ensuring compliance with Texas Medicaid, TAC rules, and professional standards.

Responsibilities

  • Conduct initial evaluations of individuals referred for PT services under HCS.
  • Develop a professional assessment identifying strengths, limitations, and therapeutic needs.
  • Determine the medical necessity of PT services and document findings in the client record.
  • Contribute to the Individualized Plan of Care (IPC) and Person-Directed Plan (PDP), as required by 26 TAC Chapter 263.
  • Write therapy objectives that are functional, measurable, and aligned with HCS waiver requirements.
  • Recommend the frequency, duration, and scope of PT services.
  • Recommend, fit, and train individuals and caregivers on the use of durable medical equipment (DME) (e.g., wheelchairs, walkers, standers).
  • Assess the need for minor home modifications that improve accessibility and safety.
  • Work with vendors to ensure proper fitting, delivery, and safe use of equipment
  • Maintain progress notes and service delivery logs in compliance with HHSC (Health and Human Services Commission) standards.
  • Update quarterly or annual re-evaluations to justify ongoing therapy.
  • Document any changes in condition and report concerns to the Service Coordinator and interdisciplinary team.
  • Ensure all PT services are billable and Medicaid-compliant under HCS program rules.
  • Adhere to Texas Board of Physical Therapy Examiners (TBPTE) rules and standards of practice.
  • Follow HCS provider policies, HIPAA, and rights of individuals under 26 TAC Chapter 263, Subchapter C.
  • Ensure therapy services respect the rights, dignity, and preferences of individuals.


Qualifications

  • Current state licensing for physical therapy assistance required
  • Excellent communication and interpersonal skills
  • The ability to create a positive, nurturing environment
  • Familiarity with Microsoft Office suite
  • The ability to work well and make decisions in a high-stress environment
