Job Overview

The Health Navigator serves as an intermediary between the health care system, social services and the community. Navigators facilitate access to services and assist families obtain the care and resources needed to maintain/improve their health. Navigators address the barriers that prevent timely treatment by identifying needs and directing patients to sources of clinical, financial, or social support.

The Health Navigator will be responsible for assessing patients, developing care plans, and making referrals to appropriate resources. This role is part of the High-Risk Asthma team in Colorado Springs and will work collaboratively to support the needs of patients and their families.

The position focuses on asthma care and spans two settings: the Asthma Outpatient Navigation program and the Home Asthma Program, which provides services in patient homes.

Department Name: Briargate Pulmonary

Job Status: 20 hours/week

Shift: Tuesday and Thursdays 8:00AM to 4:30PM and Wednesday 12:00PM to 4:00PM. There are currently no expected obligations over holidays or weekends.

Note: T his is a grant-funded position. This means the role is supported by third-party grants and/or restricted funding, and its duration is dependent on the availability of those funds. The position will be reviewed and may be extended based on the terms of the grant. Please note, if the position is discontinued or eliminated for any reason, it will not be eligible for severance.

Qualifications

EDUCATION – High School Diploma or equivalent.

EXPERIENCE – Minimum 3 years of experience assisting patients and families or other related healthcare/community experience. Knowledge of State social service agencies and community resources.

EQUIVALENCY – An Associate degree, in a related field, may substitute for the experience requirements on a year for year basis.

Essential Functions

An employee in this position may be called upon to do any or all the following essential functions. These examples do not include all the functions which the employee may be expected to perform.

  • Assesses patient needs to customize appropriate internal and external resources. Assists families in community and home settings as determined by established algorithm to remove barriers related to access services.
  • Refers families to clinical, social, and financial supports using an established algorithm to assess patient and family needs.
  • Works with staff and providers to create a care plan and communicates with them regarding the status of referrals and outreach efforts.
  • Works with individuals from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions. Builds family capacity to address health and social issues through education, care coordination and community outreach.
  • Provides advocacy and support as needed to remove barriers, preventing families from receiving services or entitlements as needed. Identifies trends in barriers experienced by families when seeking care and provides data to internal team members.
  • Maintains reports, provide necessary documents and data by deadlines and upon request.
  • Assists in meeting population health goals by linking families and communities to essential resources.
  • May research and classify community resources that will be of value to families and maintains a community resource database.
  • May represents CHCO in local, state, regional efforts related to community health and community resource needs.
  • By position may perform lead duties for other CHL team members which includes, but is not limited to, training, and managing the flow of day-to-day operations.
