The Nurse Liaison plays a critical role in coordinating the transition of patients from hospital or healthcare facilities to home-based or alternate site infusion therapy. This position works closely with hospital discharge planners, case managers, physicians, and the internal clinical and operational teams to ensure safe, timely, and seamless discharges. The Nurse Liaison also provides direct bedside education and training to patients and caregivers on infusion therapy protocols, device use, and care coordination. Responsibilities Include:
- Collaborate with physicians, case managers, and discharge planners to identify eligible patients for home or alternate site infusion therapy.
- Perform bedside visits to assess clinical needs, suitability for home or alternate site therapy, and support discharge readiness.
- Facilitate timely coordination between the hospital team and internal departments (pharmacy, operations, reimbursement) to ensure all logistics are in place for discharge.
- Deliver personalized bedside education to patients and caregivers on IV therapy administration, care of central lines, and safety protocols.
- Provide training on infusion devices and techniques, verifying readiness before discharge.
- Educate home health agency staff when needed to ensure proper care delivery post-discharge.
- Conduct initial patient assessments to confirm appropriateness for home or alternate site care and ensure alignment with payer requirements.
- Collect and communicate clinical documentation necessary for treatment planning and reimbursement.
- Document all patient interactions, education, coordination efforts, and care plan updates in a timely and accurate manner.
- Serve as a clinical liaison between the hospital and the Company, promoting services, addressing questions, and fostering trust with patients and referral sources.
- Advocate for the patient’s needs and ensure clear communication across the care team.
- Maintain ongoing contact with patients during their hospitalization and early stages of home or alternate site therapy to support adherence and address challenges.
- Monitor referral platforms and maintain communication threads with internal teams.
- Coordinate and attend meetings with hospital personnel and internal departments as needed.
- Support marketing and business development efforts by identifying new referral sources and participating in hospital-based outreach.
- Current Registered Nurse (RN) license, in good standing, required
- Minimum 2–5 years of nursing experience; home infusion, case management, or acute care preferred
- Demonstrated experience in patient education and discharge planning
- Strong clinical judgment, interpersonal communication, and organizational skills
- Proficiency with EMR systems and Microsoft Office Suite
- Valid driver’s license and reliable transportation required
