As our organization continues to grow, we are currently seeking a compassionate and detail-oriented Registered Nurse (RN) to join our Continuous Skilled Nursing Care program. This Part-time position requires a nurse visit of more than two continuous hours of nursing services per day to remain in the community.
Location: Massachusetts (statewide travel required)
About the Role:
The nurse may provide safe and effective skilled and unskilled care ranges between 2 hours and may be up to 16 hours hours continuous per day to the Complex Care patient at the comfort of their residence. Some nursing services may include (for example, Feeding tubes, Seizure care and prevention, Complex and extensive medication and treatment profiles, Tracheostomy care, Oxygen-dependence, Ventilator-dependence, intravenous and intramuscular injections).
In addition, the RN is responsible for maintaining all related documentation, coordinating care needs, and ensuring each patient meets the criteria for continued program eligibility. This includes clinical assessments, care planning, caregiver training, and collaboration with the multidisciplinary team and healthcare providers.

- Providing skilled and unskilled care needs to the complex care patient
- Assessing and continuously monitoring patients' conditions
- Administering medications and treatments as ordered by physicians
- Operating and maintaining medical equipment available to the patient
- Collaborating with a multidisciplinary team to develop and execute patient care plans
- Educating patients and their families about health conditions and post-treatment care
- Responding to medical emergencies and providing advanced life support
- Documenting patient care and updating medical records accurately
- Providing emotional support to patients and families
- Participating in continuous professional development and training
- Associate or Bachelor’s degree in Nursing (BSN preferred).
- Active RN license in good standing with the Massachusetts Board of Registration in Nursing.
- Previous home health or community-based nursing experience.
- Strong organizational skills and the ability to manage caseload
- Salary: Competitive and Negotiable based on experience
