Your Role As An RN Case Manager
- Travel within a 40-mile service area radius of the Bedford/Lynchburg office location for patient visits with generous mileage reimbursement paid out bi-weekly.
- Coordinate and oversee care for hospice patients, ensuring comfort and dignity
- Develop individualized care plans and collaborate with an interdisciplinary team
- Provide education and emotional support to patients and families
- Advocate for patients’ needs and ensure high-quality, compassionate care
- Provide direct patient care in homes and contracted facilities
- Active RN License (Virginia)
- Reliable form of transportation
- Hospice or home health experience preferred, but not required
- Strong clinical and assessment skills
- Compassion, empathy, and a heart for serving others
