Depending on assigned role, utilizes the Strengthening Families Protective Factors Framework and a variety of evidence-based practices to help families navigate the resources needed to become healthy, stable and safe.
Essential Duties And Responsibilities
Education & Experience
High School Diploma or equivalent and three years of related work experience; or equivalent combination of education and experience. Two years of college in Human Services, Business Administration, Health Care, or related field (preferred)

Essential Duties And Responsibilities
- Assist participants in scheduling prenatal appointments and provide interpretation/transportation services.
- Make initial contact with participant to present available services, provide referrals, and provide options for health care coverage and application completion.
- Create all necessary documentation accurately and timely; assist participants in developing their individualized goal/plan.
- Execute prenatal programming and education.
- Maintain a case load deemed appropriate by supervisor, which requires effective time management skills.
- Provide medical coverage information and perinatal options.
- Objectively provide culturally and linguistically appropriate information. Provide nonjudgmental, participant-centered support, recognizing that individuals are the experts in their own lives.
- Maintain and enter participant data accurately and timely in applicable databases. Assist participant with account creation and navigation of Colorado Peak Interface.
- Assist participants with SNAP education and enrollments.
- Maintain up-to-date knowledge and information on community resources to refer participants to internal and external resources as appropriate (Women, Infants, and Children, Nurse-Family Partnership, Parents as Teachers).
- Obtain necessary and ongoing training for services being provided, complete and maintain certifications as required.
- Provide information on smoking cessation, safe car seat use, infant nutrition, and mental health resources for perinatal participants.
- Actively participate in outreach/events for participants and collaborate with relevant agencies.
Education & Experience

CERTIFICATES, LICENSES & REGISTRATIONS
- Presumptive Eligibility Certification (w/in 1 year of hire)
- Application Assistance Site Certification (w/in 6 months of hire)
- Baby Me Tobacco Free Certification (Preferred)
- Child Passenger Safety Technician Certification (Preferred)
- CSFA 2.0 (w/in 30 days of hire)
- Valid Colorado Driver’s License (Required)

- The starting wage range for this role is $23.00 to $24.15 per hour.
