Position Summary
The Occupational Therapist (OT) provides medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional standards of practice.
You’ll collaborate with an interdisciplinary team to coordinate care delivery and ensure the highest level of patient outcomes.
Key Responsibilities
- Develop and implement individualized patient occupational therapy plans of care
- Conduct in-home visits for assessments and evaluations
- Communicate with physicians and care team members as needed
- Deliver education and training to patients, families, and staff
- Participate in interdisciplinary team meetings to coordinate and review care plans
- Ensure timely and accurate completion of electronic documentation
- Apply agency policies, as well as federal and state regulatory standards
- Continuously evaluate patients’ progress and update care plans accordingly
- Current Occupational Therapist (OT) license in the state of Florida (in good standing)
- Knowledge of occupational therapy principles, methods, and practices
- Excellent communication and documentation skills
- Strong problem-solving and clinical decision-making abilities
- Ability to collaborate effectively within a multidisciplinary team
- Minimum 1 year of home health or multidisciplinary OT experience preferred
