Job Overview – Behavioral Health Case Manager (Bachelor’s Level):

Compensation: $24/hour

Location: Philadelphia, PA

Schedule: Monday to Friday, 9:00 AM to 5:00 PM


Support individuals experiencing serious mental illness and housing instability as a Behavioral Health Case Manager (Bachelor’s Level) with our client in Philadelphia, PA. In this primarily field-based position, you’ll provide hands-on support in the community, connect clients to essential services, and create care plans that promote long-term stability and independence. This opportunity is ideal for early-career professionals passionate about behavioral health, outreach work, and serving high-need populations.


Responsibilities as the Behavioral Health Case Manager:

  • Field-Based Support: Conduct outreach and provide direct services to clients with severe mental illness and housing instability.
  • Service Coordination: Link clients to behavioral health care, housing resources, and community supports.
  • Care Planning: Develop and adjust individualized service plans based on client needs and recovery goals.
  • Client Advocacy: Foster engagement and assist clients in navigating complex behavioral health systems.
  • Documentation & Compliance: Complete timely records and documentation per agency and contract standards.
  • Team Collaboration: Partner with internal staff and external providers to deliver coordinated, wraparound care.


Qualifications for the Behavioral Health Case Manager:

  • Education: Bachelor’s degree in Social Work, Psychology, or related human services field required.
  • Experience: Experience in behavioral health, case management, or outreach required, with preference for work with homeless or high-need populations.
  • Technical Skills: Proficiency in Microsoft Office and case management platforms, with strong written documentation skills.
  • Industry Knowledge: Understanding of mental health, substance use, and housing systems, including trauma-informed care practices.
  • Skills & Attributes: Compassionate, self-directed, and able to build strong professional relationships in a community-based setting.
  • Additional: Valid driver’s license and access to reliable transportation is required for field visits throughout Philadelphia.
