Description

Job Title: Family Reunification Worker

Unit/Service: Family Preservation Services

Reports To: Family Reunification Supervisor

Supervises: N/A

Position Summary: Within the framework of the agency’s mission, policies, procedures and resources, the Family Reunification Case Manager is responsible for delivering case management services to assigned children and families and working as member of a team. All duties performed in compliance with current State of Michigan Licensing Rules for Child Placing Agencies, contractual obligations, and any additional applicable accreditation standards relevant to the agency.

Requirements

Qualifications:

  • Bachelor’s degree in social work, Psychology, Guidance and Counseling or other Human Services area;
  • ability to engage families in a non-judgmental manner, cultural and ethnic sensitivity, view families from a strength-based perspective, demonstrated ability to effectively provide case management services to assigned families as a member of a team, willingness to work a flexible schedule to the meet the program and individual family needs.
  • Demonstrate a commitment to diversity, equity, and inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias.

Major Responsibilities

  • Home Based Service Delivery
  • Carries caseload of 6 families dedicating a minimum of 4 hours per week of face-to-face contact with each family.
  • Makes face-to-face contact and completes initial intake within 5 days of referral, per contract requirement.
  • Provides back up coverage for the team.
  • Provides timely delivery of services adhering to the Solution Focused/Strength Based Model.
  • Available 24 hours a day, 7 days per week, to meet the family’s needs and schedules
  • Follow up services are provided to all families at 6 & 12 months as required.
  • Documentation
  • Completes all case file documentation according to established time frames and in accordance with contract requirements, accreditation standards, and licensing rules.
  • Content of case notes and reports accurately reflect service/case activity.
  • Meetings/Trainings
  • Attends and participates in Team Meetings on a weekly basis.
  • Attends and participates in Team Supervision weekly and Individual Supervision as scheduled.
  • Attends outside and internal agency meetings and trainings as scheduled and/or required.
  • Utilization of Agency Resources
  • Adheres to the Family Reunification budget re: Flex Funds and keeps within the $500 per family average.
  • Effectively utilizes and explores community resources with families.
  • Other tasks as assigned.
  • Maintains professionalism in all interactions with families, DHHS and other community agencies.
  • Takes the initiative to increase overall service delivery to improve the Family Preservation Program.
  • Other tasks/duties.
