Job Title: Outreach Coordinator

Work Location: Greater Holyoke Area

Duties and Responsibilities:

  • Assist HPD Intervention Officers with monitoring police radio.
  • Respond to calls for service with Intervention Officer.
  • Determine recovery needs of individuals at the scene.
  • Make initial contact with potential clients at the scene by describing the peer-to-peer recovery support and other services offered by ERASE 2 program.
  • As appropriate, arrange for follow up contact with client.
  • As appropriate, coordinate with emergency personnel at the scene to connect client to detox, medical, or behavioral health services.
  • Use StreetReach App to track activities.
  • Monitor activities of the other members

Minimum Qualifications:

  • Bachelor’s degree in social work, psychology, nursing, or related field.
  • Valid driver’s license and automobile with insurance. Skills, Knowledge and Prior Experience Required:
  • Knowledge of behavioral health and medical services, resources and supports for housing, vocational training and employment, harm reduction and recovery services.
  • An elevated level of skill in organization, time management, ability to multitask and work independently.
  • Basic computer skills.
  • Ability to communicate professionally and accurately.
  • Ability to work well with individuals from diverse economic, racial, and ethnic backgrounds, cultural competency.
  • Experience in human services case management preferred.
  • Fluency in Spanish, spoken and written, preferred. Supervisory Relationships:
  • Reports to supervisor at employer (culturally specific organization). Collaborates regularly with Project Director
