Work Location: Greater Holyoke Area

- Assist HPD Intervention Officers with monitoring police radio.
- Respond to calls for service with Intervention Officer.
- Determine recovery needs of individuals at the scene.
- Make initial contact with potential clients at the scene by describing the peer-to-peer recovery support and other services offered by ERASE 2 program.
- As appropriate, arrange for follow up contact with client.
- As appropriate, coordinate with emergency personnel at the scene to connect client to detox, medical, or behavioral health services.
- Use StreetReach App to track activities.
- Monitor activities of the other members
- Bachelor’s degree in social work, psychology, nursing, or related field.
- Valid driver’s license and automobile with insurance. Skills, Knowledge and Prior Experience Required:
- Knowledge of behavioral health and medical services, resources and supports for housing, vocational training and employment, harm reduction and recovery services.
- An elevated level of skill in organization, time management, ability to multitask and work independently.
- Basic computer skills.
- Ability to communicate professionally and accurately.
- Ability to work well with individuals from diverse economic, racial, and ethnic backgrounds, cultural competency.
- Experience in human services case management preferred.
- Fluency in Spanish, spoken and written, preferred. Supervisory Relationships:
- Reports to supervisor at employer (culturally specific organization). Collaborates regularly with Project Director
