- Develop and follow individualized care plans
- Accurately document patient care, observations, and updates
- Collaborate with interdisciplinary care teams
- Maintain compliance with DOL, CMS, and state regulatory requirements
- Current LPN license in the state of [WA]
- Current CPR certification
- Valid driver's license and reliable, insured vehicle
- Home health experience preferred but not required
- Must pass pre-employment screenings
