Job Summary
The Quality Improvement (QI) Nurse is an integral part of the Quality Improvement team. The nurse works closely with various departments to identify initiatives to enhance individual and overall health through education, outreach, and direct delivery of preventive care. This includes helping to close care gaps to improve HEDIS, UDS measures as well as other PCMH activities. The QI Nurse collaborates on initiatives related to care management.

Qualifications/Requirements
- Graduation from an accredited LPN program.
- Clear active Florida license as an LPN or RN.
- Skills in providing health screenings and preventive care.
- Ability to work effectively with a diverse multidisciplinary team of health care professionals and support staff.
- Ability to manage multiple and simultaneous responsibilities and to prioritize scheduling of work.
- Excellent internal and external customer service skills.
- Knowledge of adult learning methodologies and ability to engage patients in improving their health through education and support.
- Computer skills including Microsoft Office applications.
- Must demonstrate personal and professional commitment to the mission of Heart of Florida Health Center and its Mission Statement and values.
- Identify patients who are due or overdue for preventive care, focusing on chronic care conditions including but not limited to diabetes and hypertension.
- Engage and educate patients on healthy lifestyles and the importance of regular preventive care. This includes reviewing medical histories, 1:1 assessment and counseling, documentation of services, and follow up as appropriate.
- Participate in delivery of evidence-based patient education programs related to management of chronic illnesses.
- Train and coach other staff involved in patient health education, as appropriate.
- Participate in HFHC health outreach activities.
- Participation in the support of the care team through chronic care disease management activities.
- Participate in team meetings for Quality Improvement and assist in projects to improve systems and processes and outcome measures.
- Attend team meetings at clinics on a regular basis.
- Participate in various committees when requested.
- Work with patients to meet various clinical quality measures, including those related to grants, by ensuring accurate follow-up and data collection.
- Participate in multi-organizational learning collaboratives related to population health, to improve service delivery and outcomes by implementing current and best practices.
- Other duties and tasks as assigned.
- While performing the duties of this job, the employee is regularly required to talk and hear.
- Ability to lift 20 pounds.
- Must be able to lift, carry and handle equipment, supplies and other work site materials according to position requirements.
