Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health Hospice team. In this role, you will provide skilled nursing care and support to terminally ill patients in their homes, helping to ensure comfort, dignity, and quality of life. You will collaborate with an interdisciplinary team to develop and implement individualized care plans based on each patient’s unique needs and goals.
Key Responsibilities
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health Hospice team. In this role, you will provide skilled nursing care and support to terminally ill patients in their homes, helping to ensure comfort, dignity, and quality of life. You will collaborate with an interdisciplinary team to develop and implement individualized care plans based on each patient’s unique needs and goals.
Key Responsibilities
- Provide direct nursing care to hospice patients in their homes, including assessments, symptom management, and end-of-life care.
- Develop and implement individualized care plans in coordination with physicians, social workers, chaplains, and other healthcare professionals.
- Administer medications and treatments in accordance with physician orders and hospice care protocols.
- Educate patients and families on disease progression, symptom management, and the dying process.
- Document patient care accurately and in a timely manner using the electronic medical record (EMR) system.
- Participate in on-call rotation and provide after-hours care as needed.
- Ensure compliance with all federal, state, and local regulations as well as organizational policies.
- Active and unrestricted Registered Nurse (RN) license in the state of practice.
- Minimum of 1 year of nursing experience, preferably in hospice, palliative care, or home health.
- Excellent clinical assessment, communication, and critical thinking skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and a valid driver's license.
- Prior experience in hospice or palliative care nursing.
- Knowledge of Medicare/Medicaid hospice regulations and documentation standards.
- Familiarity with electronic medical record (EMR) systems.
- Current Basic Life Support (BLS) certification.
- Hospice and Palliative Nurse (CHPN) certification is a plus.
