Position Summary

The Psychosocial Educator identifies educational needs of psychosocial staff and develops, implements, evaluates, and revises educational programs. This position also coordinates and enforces quality improvement initiatives, provides guidance to staff members as needed, and promotes a positive team culture.

Schedule: Full-time Monday - Friday. Requires availability from 7:00 a.m. to 5:00 p.m., with standard working hours of 8:00 a.m. to 4:30 p.m. With occasional weekend hours. Weekends are about every 10–12 weeks, and the administrative call starts at 5:00 p.m. on Friday and ends at 7:00 a.m. on Monday. On-call rotation occurs every 3–4 weeks and includes one weeknight of administrative call from 5:00 p.m. – 7:00 a.m. One holiday rotation every other year.

Key Job Duties

Clinical Performance:

  • Develop, coach, and mentor skills through hands-on teaching, competency audits, practice observations, and documentation reviews.
  • Conduct assessments, and address performance issues with plans that include feedback, coaching, training, and monitoring progress.
  • Assist in the supervision of psychosocial staff members, providing guidance and education when needed.

Patient Experience & Quality

  • Ensure a high level of quality care is provided to patients and families.
  • Execute on identified process improvement opportunities and create initiatives as necessary.
  • Develop and maintain team specific best practices.
  • Participate in project and program development and ownership as it aligns with departmental and organizational strategy.
  • Participate in workgroups to improve or enhance care programs or care delivery.

Software Subject Matter Expert

  • Oversight of the Electronic Medical Record efficiency and effectiveness in achieving desired quality, billing, documentation and compliance outcomes.
  • Train on EMR software.
  • Field and address software-related questions from PS Staff (Learning Management System, EMR, Intranet).
  • Obtain and maintain relevant software competencies and credentials, as applicable.

Education

  • Develop ongoing education plan including providing and communicating CEU opportunities.
  • Function as clinical resource to staff.
  • Participate in staff education such as New Employee Orientation, Skills Fair, etc.
  • Develop and maintain competencies to ensure compliance and accuracy.
  • Assist in ensuring education meets Unity’s regulatory requirements and best practices.
  • Maintain updated best practices for psychosocial disciplines.
  • Assess and provide mentoring to Psychosocial staff.
  • Provide educational outreach to community partners.
  • Collaborate with leaders to ensure staff are meeting expectations.
  • Participate in evaluation visits to ensure competency of staff members.
  • Work collaboratively with volunteer services to orient new volunteers.

Leadership

  • Model and drive mission, vision, values and culture while monitoring and managing individual and team morale.
  • Support employees’ career growth by understanding their personal and professional development goals.
  • Provide communication to staff regarding organizational changes and processes.
  • Attend networking events.
  • Maintain connections with state and national hospice collaboratives as needed.
  • Assist in Scheduling Coordination when needed.
  • Rotate through administrative call coverage.
  • Assist with interviews for the psychosocial team members.
  • Maintain a physical presence at all Unity locations.
  • Ability to work evenings, overnight on-call, and weekends as needed.

Knowledge, Skills, And Abilities

  • Computer proficiency (MS Office – Word, Excel, and Outlook)
  • Highly motivated and organized with a strong attention to detail and analytical skills to help solve problems.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment.
  • Ability to keep up to date with policies, practices, and regulations.
  • Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures.
  • Comply with all applicable local, state, and federal laws and regulations as well as policies and procedures of Unity.
  • Satisfy mandatory training requirements regarding local, state, and federal regulations and/or Unity’s policies and procedures.
  • Perform other job-related duties as assigned.
  • Ability to adjust to frequently changing demands.
  • Strong organizational and analytical skills with attention to detail.

Requirements

  • Bachelor’s degree in a Social Services related field (Social Work, Theology, etc.) Master’s degree in Social Services related field preferred.
  • If a Social Worker, appropriate licensure to practice in the State of Wisconsin, if applicable. (CSW, CAPSW, CISW, or LCSW)
  • Minimum of 2 years project participation and execution, or leadership experience.
  • Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
